Purpose:
|
The recipient proposes to apply the Selkirk Team for At-Risk Teens (START) program model to youth involved in the justice system diagnosed with cognitive deficiencies or mental health issues. START is a multi-agency program that targets youth with complex challenges and involvement with multiple social service agencies that aims to implement a network of supports for each client and provide coordinated services from the agencies involved. The proposed model will provide weekly on-going assistance to youth through a plan that will be created that focuses on the youth's strengths and supports their weaknesses. The approach will also provide on-going communication and case management to the youth and their family.
